Providence Case Manager - Mental Health Services in Valdez, Alaska
Description:
Providence is calling an Case Manager (Mental Health Svc DMHDD, Day Shift, 0.5 FTE) to Providence Valdez Counseling Center in Valdez, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Develop a mutual and reciprocal relationship with clients and their families when appropropriate based upon respect for the uniqueness of each individual. Provide a structured environment and maintains therapeutic rapport.
Conduct comprehensive, individualized psychosocial (functional) assessments with clients to determine case management interventions, symptom control, rehabilitation needs, and crisis intervention. Develops an individual written service plan which identifies treatment needs based on client identified problems. Work closely with the Behavioral Health Clinicians and Peer Support Specialists to implement specific interventions as outlined in the treatment service plans.
Coordinate all services identified in the psychosocial (functional) assessment and service plan, linking clients with other providers and community agencies as necessary.
Deliver mental health rehabilitative services, including case management, individual development, family skill development, group skill development and recipient support services. Services will be provided in the most appropriate location.
Provide a reliable communication link to other rehabilitation staff, outpatient therapy staff and staff psychiatrist regarding clients on their caseload. Function as a member of the Center interdisciplinary treatment team. Provides a reliable communication link to Business Office staff and the Center Manager regarding client financial and living arrangements.
Provide skills training, psycho-education groups, and support as needed and as outlined in the individualized service and rehabilitation plans.
Provide documentation of all client contact in accordance with state regulations and Center policy. Participate in the continuous quality improvement process when requested. Maintain a current status of all client files. Review service providers for updated documents and ensures that each client maintains a current status with all service providers and entitlement agencies.
Comply with all Center policies and procedures. Provide client transportation utilizing Center vehicles as required.
Implement care/services that recognize age/diversity specific needs/issues of customers served.
Qualifications:
Required qualifications for this position include:
Bachelor's Degree
Basic Life Support and First Aid certification required within 90 days of hire
Alaska driver's license is required and must be obtained within 30 days of hire
Requires knowledge of mental health issues, both theory and practice; crisis intervention techniques; assessment techniques, and family systems dynamics
Requires strong crisis intervention, assessment, interpersonal communication, and organizational skills
Must be able to monitor and shape client behavior relative to individual goals and objectives as identified in the treatment plan, document all client contact clearly and accurately; be flexible and able to function well in a constantly changing environment, respond constructively to supervisory personnel and establish and maintain positive, effective working relationships with the PVCC team, other staff and the public.
Knowledge of data entry and word processing
Preferred qualifications for this position include:
Bachelor's Degree in behavioral health or education field
Experience working in a behavioral health
