Providence RN Care Specialist - Pediatric Center for Congenital Heart Disease in Spokane, Washington

Providence is calling a Registered Nurse (RN) - Care Specialist (Pediatric Center for Congenital Heart Disease , Day Shift, Full Time) to Providence Medical Group in Spokane, Washington. In this position you will:
Provide leadership and continuity of care guidance for patients with chronic diseases and/or complex disease states. Use knowledge of illness, diagnoses and treatments in comprehensive, holistic assessments of patients.
Provide teaching to patients/families related to patient’s diagnosis, pathology, medical and nursing treatment plans, discharge needs and health goals; documents each element of care per organizational and unit standards.
Utilize appropriate resources to gather data and assess patient care needs; make ongoing assessments of the patient including but not limited to: physio-biological, cognitive/emotional, spiritual and social, managing life with chronic disease.
Identify possible problems that could lead to an emergency/crisis situation and takes appropriate action to de-escalate the potential for the situation to occur.
Maintain complete documentation of all critical and essential aspects of medical history and information in the patient’s file. Manage patient related activities in an outpatient, ambulatory clinic and/or inpatient, acute care setting.
Assist in designing and implementing planned care interventions through a combination of evidence-based guideline/pathway utilization and individualized care planning.
Mobilize resources and manages the system in response to patient need according to an interdisciplinary plan of care.
Qualifications:
Required qualifications for this position include:
Graduation from an approved, accredited school of nursing
Currently licensed as a Registered Nurse (RN) in the State of Washington
2 years of work experience in an RN role
Work experience revolving around medications and IV solutions in terms of purpose, administration, side effects and contraindications and administers according to established standards and provider orders
Experience managing complex patients in an ambulatory setting
Preferred qualifications for this position include:
- Bachelor's Degree in Nursing
