Description:

Providence is calling a Diabetes Educator (Community Wellness, Days, 0.6 FTE, 24 hours per week) to Providence Health Care Region in Spokane, Washington. In this position you will:

  • Apply expert knowledge along with extensive related experience and judgment to accomplish goals and objectives.

  • In-depth knowledge about current diabetes treatment management.

  • Able to lead and effectively manage groups.

  • Ability to develop a collaborative, therapeutic alliance with individuals.

  • Collect DSME/T program participant assessment data, using the ADCES7 framework, in a collaborative and ongoing manner.

  • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME/T program participants.

  • Provide educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.

  • Collaboratively develops an individualized follow-up plan with each program participant.

  • Evaluate effectiveness of educational services provided by measuring attainment of learning objectives.

  • Conduct a follow-up assessment upon completion of DSME/T program services, using outcome measures from the ADCES7 Continuum of Outcomes Framework.

  • Document assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.

  • Utilize a team approach to provide services and collaborates and communicates with team members when needed.

  • Identify when a program participant’s needs are outside the scope of the instructor’s practice and expertise, plus arranges for additional services to meet needs.

  • Participate in the development of training materials.

  • Contribute to, and participates in, a continuous quality improvement process to measure DSME/T program and to identify and address opportunities for improvement.

  • Appraise his/her performance to identify areas of strength and areas for improvement and to develop a plan for improvement and growth.

  • Participate in peer review process to evaluate performance of other professional instructional staff.

  • Maintain 15 hours of continuing education annually specific to diabetes, diabetes related topics and behavior change and self-management education strategies.

  • Comply with INHS policies/procedures regarding mandatory in-service training/education, safety and personnel practices.

  • Responsible for demonstrating an awareness and sensitivity to information, ensuring privacy and confidentiality. Knows basic HIPAA law as it pertains to health plans. All employees of Inland Northwest Health Services who have direct or indirect access to patient health information must treat that information with the strictest confidence. Any unauthorized access, modification, disclosure or casual discussion of such information shall be considered a violation of policy and shall be the grounds for disciplinary action up to and including termination of employment.

Qualifications:

Required qualifications for this position include:

  • Education to meet certification, license or registration requirement.

  • A healthcare professional who is a Certified Diabetes Care and Education Specialist (CDCES) or preparing for the NCBDE qualifying examination.

  • Six (6) months experience providing diabetes education and/or 15 contact hours within past year in diabetes and diabetes clinical and educational subjects.

Preferred qualifications for this position include:

  • Credentialed as a diabetes education specialist or diabetes clinical management specialist.

  • CDE certified.

  • Three (3) years experience as a healthcare professional.
