Swedish Health Services Social Worker - Multiple Sclerosis Center in Seattle, Washington
Swedish Medical Center is looking for a Social Worker (0.0 FTE, Days) to work in Multiple Sclerosis Center at Swedish Medical Group in Seattle, WA.
Participates in the inpatient or outpatient activities. Engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making. Provides direct patient care as well as consultative services to assure the delivery of state-of- the-art care.

Social Worker is guided by the NASW Code of Ethics and the Swedish Mission and Vision in daily practice.
Deliver upon the service expectations of both our patients and fellow staff members by listening to their needs; engaging in positive interactions; and following through on promises made in a thoughtful, efficient, timely and courteous manner so that their total outcome is better than expected.
Respect the dignity, confidentiality and privacy of patients.
Work in a safe manner, adhering to general safety precautions and standards. Report any unsafe conditions to their supervisor and/or the safety hotline.
Asses psychosocial needs for specific patient populations using advanced practice care and hospice knowledge and skills:
Obtain psychosocial history.
Identify patient short and long term goals.
Identify available family and social support.
Identify discharge options related to patient diagnosis, prognosis and identified care giving needs.
Required qualifications for this position include:
Master's Degree in Social Work
Licensed Independent Clinical Social Worker (LICSW)
Advanced training, geriatric or medical social work
3 years Inpatient, outpatient, or home-based social work experience
