Swedish Health Services Social Worker Maternal and Fetal Specialty Center in Seattle, Washington
Swedish Medical Center is looking for a Social Worker (0.5 FTE, Days) to work in the Maternal Fetal Specialty Center at Swedish First Hill in Seattle, WA.
The Maternal and Fetal Specialty Center Social Worker is responsible for providing medical related social services to patients and families. These services may include but are not limited to, advocacy for patient rights, assessment and care planning related to mental and psychosocial needs. Responsible for short-term outpatient therapy, grief and loss support, linkage with community resources, supportive counseling and crisis intervention with patients and in conjunction with multidisciplinary team members. Works with pediatric specialty care providers to coordinate combined care conferences.
In this position you will:
Provides supportive counseling, short-term outpatient therapy, consultation and crisis intervention
Coordinates care across the Swedish system, including Inpatient nursing staff prior to discharge, Patient Registration, Perinatal Bonding and Support Team, the Lytle Center, OR for disposition planning, and physicians
Provides grief counseling, resources, and disposition planning to patients experiencing severe fetal diagnoses and/or demise
Communicates effectively and in a timely manner with patients, families, co-workers and community agencies.
Communicates interdisciplinary plan of care to patients and families.Assesses psychosocial functioning and social work needs for patients, families, and/or their support systems.
Required qualifications for this position include:
Master's Degree in Social Work from an accredited school required
Licensed Social Work Associate – Advanced (LSWAA) or Licensed Social Work Associate- Independent Clinical (LSWAIC) required
2 years experience in clinical setting
Wide range of knowledge in areas of psychosocial assessment, resource, and referral
Preferred qualifications for this position include:
- Licensed Advanced Social Worker (LASW) or Licensed Independent Clinical Social Worker (LICSW) with the Washington State Department of Health under Chapter 18.225 RCW preferred
