Swedish Health Services Senior Patient Care Coordinator- Access Services First Hill in Seattle, Washington
Swedish Medical Group is looking for a Senior Patient Care Coordinator (1.0 FTE, Day shift) to work in Access Services First Hill at Swedish Medical Group in Seattle, WA.
This position is responsible for the overall coordination of seamless care for patients in a surgical and/or non-surgical setting. They will serve as a mentor and resource to Patient Care Coordinators by providing support, guidance and training as needed.
In this position you will:
Transmit work instructions from the Supervisor/Manager to the staff, assign workload duties to the staff as approved by the Supervisor, ensure transactions are logged and keyed accurately and on a timely basis providing a smooth flow of work through the department and notify the Supervisor/Manager of backlogs.
Evaluate the efficiencies of the team as well as identifying barriers and move to eliminate them where possible.
Assist with monitoring performance and relaying feedback to caregivers and supervisors.
Act as a resource for staff members, troubleshoot most difficult problems within the department and find appropriate solutions.
Work closely with Supervisor/Manager to continually update and improve functions.
Required qualifications for this position include:
High school diploma or GED
Two (2) years experience in healthcare setting with increased responsibilities or one (1) year experience in Patient Care Coordinator type role
Preferred qualifications for this position include:
- Electronic Medical Records (EMR) experience
