Swedish Health Services RN Navigator Referrals - Administration in Seattle, Washington
Swedish Medical Group is looking for a RN Navigator Referrals (1.0 FTE, Day Shift) to work in Administration Department at Swedish Medical Group in Seattle, WA.
Under the general supervision/direction of the Chief Medical Officer, Value-Based Care and Ambulatory Quality, the RN Navigator Referrals shall be responsible for providing direct patient care navigation improving patient experience through promoting and ensuring timely, seamless and appropriate referrals . The purpose of the Referral Navigation program is to promote quality of care, cost effective patient outcomes, efficient resource utilization, while meeting the expectations of patient, family, referring physicians and agencies, and payer expectations.

For direct patient care roles (when applicable): Performs and maintains currency of essential competencies as required by specific area of hire and populations served.
Co-lead referral quality improvement programs including developing, implementing, and refining clinical guidelines as related to referrals.
Ensure referral integrity through review, intervention and redirection based on the condition specific pathways to appropriate specialists.
Required qualifications for this position include:
Bachelor's Degree in Nursing
Licenses, Certifications and Registrations must be unencumbered and valid in the state of Washington
Applicant must have and maintain a current Washington RN license
Two (2) years Ambulatory Nursing
Preferred qualification for this position include:
- Case Management experience
