Swedish Health Services ED Social Work Case Manager - Case Management in Seattle, Washington
Swedish Medical Center is looking for a Emergency Room Social Work Case Manager (0.9 FTE, Night Shift) to work in Case Management at Swedish Shared Services both in Issaquah, WA and Seattle, WA.
Our team is seeking a flexible ED Case Manager to work in two locations (Issaquah & Seattle). The Clinical Social Worker partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The clinical social worker is responsible for providing a full range of social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The clinical social worker partners with the patient and his or her support system, as well as interdisciplinary teams, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of health care resources.
In this position you will:
Work collaboratively with patients nurses, case managers, physicians, other practitioners, caregivers and community resources and agencies. The social worker and healthcare team are jointly accountable for measurable outcomes that are cost effective and reflect patient preferences and values
Communicate patient needs related to advancing the medical treatment plan and/or discharge plan to appropriate professionals and follows up
Develop discharge plan through collaboration with the interdisciplinary team
Procure services and resources for identified patients and families, serving as an advocate to promote achievement of agreed upon goals
Advocate on behalf of patients and caregivers for service access or creation, and for the protection of the patient’s health, safety and rights
Assure prudent utilization of all resources (fiscal, human, environmental, equipment and services) by evaluating the options available and balancing cost and quality to assure the optimal clinical and financial outcomes
Accept responsibility and accountability for achievement of optimal outcomes within their scope of practice
Acquire and maintain knowledge and competence related to the expectations of their position and practices within their scope
Required qualifications for this position include:
Master's Degree of Social Work (MSW) from an accredited institution
1 year experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting
Licensed Social Work Associate – Independent Clinical (LSWAIC)
Preferred qualifications for this position include:
Licensure as an Independent Clinical Social Worker (LICSW) with the Washington State Department of Health under Chapter 18.225 RCW
