We are looking for a Volunteer Coordinator for the HCN Sonoma Hospice Indirect Department at St Joseph Home Care Network.
Location: Santa Rosa, CA
Work Schedule: On Call / Per Diem
Shift: 8-hour, Days

The Volunteer Coordinator is responsible for the orientation, training and coordination of all St. Joseph Home Care Service Hospice volunteers and for volunteer program administration and development in all volunteer areas.
Plans, coordinates, schedules and supervises delivery of all volunteer services according to the Plan of Care.
Schedules volunteers to service on the basis of Hospice patient needs and the interests and skills of the volunteers.
Organizes and reviews monthly a record-keeping system that includes services delivered and actual time involved.
Recruits, interviews, and selects compassionate and competent volunteers.
Provides and coordinates the orientation and training of volunteers as needed for patient care.
Coordinates volunteer’s compliance with Plan of Care and Network policies and procedures. Reports volunteer schedules and changes in Plan of Care to IDG.
Prepares service reports as required by the Director of Hospice.
Functions as a liaison between St. Joseph Hospice IDG and the volunteers.
Facilitates community awareness and support of St. Joseph Hospice volunteer program.
Fosters and develops relationship with other program coordinators, managers and director.
Participates in IDG weekly.
Coordinates and documents schedules and verifies visits within the EMR
Demonstrated skills in administrative functions including ability to develop policies and procedures.
Demonstrated skills with written and oral communications.
Demonstrated ability to execute a supportive and sensitive approach to volunteer and patient/family needs.
Demonstrated knowledge of end of life care.
Demonstrated knowledge of and skills in computer program (i.e. Outlook, Word, Allscripts and Excel)
Minimum Position Requirements:
Education: Bachelor's Degree or High School diploma with 4 years of equivalent experience.
2 years Supervisory experience.
1 year Volunteer hospice experience.
Current California Driver’s license.
Proof of automobile insurance
Preferred Position Qualifications:
Experience: Experience as a Volunteer Coordinator for a hospice program.
