We are looking for a Patient Placement Supervisor for the NCR Command Center Department at Santa Rosa Memorial Hospital.

Location: Santa Rosa, CA

Work Schedule: On Call / Per Diem

Shift: 12-hour, Days

Job Summary:

Reports to the Director of Capacity Command Center and is responsible for the coordination of patient flow and supervisory activities of regional float staff, staffing office, and transfer center during their assigned shift. This position serves as a nursing resource in collaboration with ministry house supervisors, and intervenes in difficult situations regarding staffing decisions, the assignment of patients, and regional referral/transfer patients, as necessary. This position has the authority and responsibility to exercise "command decisions" in directing the flow and placement of patients throughout the NorCal Region and safety-related situations. They direct the daily operational and personnel activities of designated facility(s) for Patient Placement and the NorCal Regional Transfer Center. Supports a collaborative and coordinated process among departments, clinics, physicians, and department staff to promote effective management of patient care across the healthcare continuum. Fosters teamwork and integration of services and staff in Capacity Command Center. Facilitates growth and development of the department in response to the dynamic nature of the healthcare environment. Develops programs, policies, procedures, and standards of care to ensure patients' nursing care needs are assessed, evaluated, and met.

Essential Functions:

  • Assumes responsibility for the overall ministry throughput and patient flow operations, communicating significant issues with appropriate department manager, director, nursing executive, other executive leader and/or administrator on-call.

  • Functions as bed control manager, primary contact for bed planning, staffing and patient flow. Serves as first line liaison with the transfer center, departmental staff and physicians regarding patient acceptance and or directs admissions to other regional ministries if capacity is limited based on services needed and available. Identifies appropriate placement for patient admission/transfer. Coordinates and expedites admission/transfer to avoid delays and obstacles. Works in conjunction with physicians, staff, to prioritize admissions/transfers, based on medical needs.

  • Works closely with unit managers and directors to ensure compliance with EMTALA, JCAHO, Title 22, The Joint Commission (TJC), Centers for Medicaid and Medicare (CMS) standards and regulations.

  • Utilizes the administrative and medical staff leadership chain of command as a resource to protect patient safety and manage ministry risk.

  • Responsible for working with the staffing coordinators and ministry house supervisors to assure efficient, appropriate and equitable staffing for all patient care units based on patient acuity, availability of staff and guidelines. Ensures daily staffing schedules are completed and posted in a timely manner and that staff assignment records are complete and accurate.

  • Collaborates closely with departmental managers and shift leads to ensure that census and financial targets are met.

  • Facilitates daily bed huddle meetings with managers and lead staff.

  • Responsible for the oversight of regional float pool assigned to shift.

  • Identifies and resolves system/personnel problems that could negatively affect patient flow. Follows through on problem resolution. Provides input on performance of staff house-wide and relays pertinent information to appropriate department manager.

  • Performs and supports human resource functions of assigned staff to include hiring, time and attendance, annual evaluations, counseling, terminations and annual competencies.

  • Ensures RN and other care team members demonstrate competence in assessing and interpreting age appropriate data about the patients’ status in order to provide the care needed.

  • Ensures that staff demonstrates appropriate communication skills for the patient population served.

  • Ensures that staff demonstrates knowledge of age-specific community resources

  • Strong basic working knowledge of applications, including bed tracking system.

  • Basic understanding of staffing and budgetary/fiscal concepts

Minimum Position Requirements:

Education: Graduate of an accredited school of nursing.

Experience: 3 years recent acute care clinical experience

Licenses/Certifications: Current RN license

Preferred Position Qualifications:

Education: Bachelor's Degree in Nursing

Experience:

  • 2 years House Supervisor or Management experience

  • 5 years ER or Critical Care Clinical experience
