We are looking for a Nurse Navigator for AMG SRMH Chargeback Admin at St. Joseph Heritage Healthcare.

Location: Santa Rosa, CA

Schedule: Full Time, 64 bi-weekly hours

Shift: 8-hour Days

Job Summary:

The Nurse Navigator, for Oncology is responsible for the individual patient/family assessment, referral to service and follow-up. This position is the hub of multiple lines of communication between the patient, referring physician, specialists (radiologists, surgeons, medical oncologists and radiation oncologists), staff and other support service providers. The position encompasses the roles of coordinator liaison, communicator, educator and supporter. In addition, this position contributes to the productive and efficient operations of RRMG.

Skills:

  • Basic computer skills including Microsoft Word, Windows, Excel, and an email system.

  • Possess a high level of knowledge and understanding of oncology illnesses and treatment modalities.

  • Skill in the coordination of patient’s care and needs.

  • Sensitivity to coordinate care for patients from a variety of ethnic, cultural and social economic backgrounds with varied medical and developmental needs.

  • Possess general understanding of clinic/departmental operations.

  • Possess leadership skills including group facilitation and conflict management.

  • Ability to work independently, set priories and meet deadlines.

  • Ability to function in a setting with numerous interruptions (multi-tasking skills are critical).

  • Excellent interpersonal skills necessary to interact effectively with members of the interdisciplinary team, including physicians, as well as external customers, to achieve desired clinical services and financial outcomes.

  • Ability to communicate and work effectively with the physical and emotional development of all age groups.

  • Must possess ability to be compassionate, articulate and patient.

  • Strong communicator, excellent interpersonal and organizational skills.

  • Ability to read and interpret documents such as safety rules, operating and maintenance instruction, and procedure manuals.

  • Ability to write routine reports and correspondence.

  • Ability to add, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals.

  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

  • Ability to deal with problems involving several concrete variables in standardized situations.

    Position Requirements:

    Education:

  • H.S. Diploma or GED

  • Graduate of an accredited Registered Nursing Program

    Experience:

  • Experience in the coordination of patient's care and needs.

    License/Certification:

  • Current CA RN Licensure

  • Active BLS certificate

    Preferred Position Qualifications:

    Education:

  • Bachelor's Degree in Nursing

    Experience:

  • Two (2) years recent oncology nursing experience

  • Program development experience

  • EME or IDX experience

    License/Certification:

  • OCN preferred
