Providence Health & Services Community Health Supervisor in Santa Monica, California


Providence is calling a Community Health Supervisor (Full Time/Day Shift) to Saint Johns Health Center in Santa Monica, CA.

We are seeking a Community Health Supervisor who is responsible for oversight of one or more Community Health programs. Principal duties include hiring, training, supervising, expense control, and data collection related to program evaluation. He/She is also responsible for coordination of outreach, scheduling events, department statistics, data collection and supplies. The Community Health Supervisor works as part of a team to implement programs that address the health needs identified by the Community Health Needs Assessment. He/she also develops and maintains intentional relationships with community partners and facilities partnerships with Providence staff, school districts, churches, clinics, local government and businesses, community groups, funding agencies and community members.

In this position you will have the following responsibilities:

  • Hire, supervise, appraise, train and discipline staff, in accordance with Providence policies and procedures.

  • Act as a technical knowledge resource to employees and community partners.

  • Set up and monitor schedules for the Community Health employees to provide outreach, enrollment (if applicable), screening, health education, physical activity, case management and technical assistance to clients and community organizations

  • Monitor and evaluate job performance of Community Health employees in accordance with job description, and employee's goals and objectives, including observation of employee in his/ her working environment

  • Facilitate communication among staff, at regularly held staff meetings and in daily interactions

  • Research and coordinate ongoing training and education that relates to current projects for Community Health employees

  • Supervise timely completion of paperwork requirements including service units reporting, budgetary expense and approval of time sheets and expenses.

  • Implement and monitor collection of program’s evaluation data and the ability to summarize and communicate key points to Manager

  • Participates in conducting trainings targeted at the development of Community Health employees across the entire department

  • Establish and maintain proactive communication with supervisor

  • Participate as a member of Community Health Administrative Team to review progress, initiate ideas for improvement, and align outreach to multi sector community stakeholders, community coalitions and media/social networking partners

  • Support the design, development and refinement of Community Health messaging and story lines to be disseminated to community coalitions and the general public, including the development of communication tools and social networking

  • Work closely with Manager, Community Health to ensure department goals are being met and formulates solutions to meet goals when needed

  • Develop and implement a curriculum for community education workshops related to insurance utilization.\

  • Responsible for statistics, assuring that they are entered into relevant Community Health software per department deadlines. Reviews monthly reports with Manager

  • Audit staff’s statistics two times a year

  • Document all outreach, and maintains all logs and sign-in sheets

  • Creates and maintains stock of current outreach materials, applications and handbooks.

  • Actively seek opportunities to outreach with a focus on building relationships with hard to reach populations through individuals or community safety net organizations that share the Providence Mission to serve the poor and vulnerable. Focuse on building relationships and networking to collaborate on events suitable for the Community Health department

  • Schedule outreach events and assure staffing is in place and within budget. Works with lead agency regarding needs for event (table, chairs, shade, electricity)

  • Work with other Community Health staff to coordinate events. Updates Outlook calendar with event information (time, type, location, assignments)

  • Track and monitor employees outreach activities (weekend and evening events)

  • Make presentations about available services to local community based organizations

  • Act as representative for Providence at professional coalitions and network meetings

  • Provide on-site and home-based assistance of enrollment and renewal applications to families from partner organizations (clinics, schools, churches, businesses, etc.).

  • Build relationships with parents by asking appropriate questions in a compassionate manner and uses good listening skills

  • Follow up and assists with families having difficulty with enrollment process.

  • Conduct proactive community based follow up with families to make sure they stay enrolled and renew their application on time

  • Administer client satisfaction survey to clients assisted.

  • Fully understand and is able to explain insurance programs (eligibility, application process, requirements, premiums, co-pays) and other community health services

  • Maintain professional and technical knowledge by attending educational workshops or reviewing professional publications to stay informed of eligibility and benefit changes to insurance programs

  • Work closely with Community Health enrollment/emergency room staff to assure a seamless flow for qualified uninsured adults to gain access to Medi Cal, CalFresh, or Covered CA

  • Responsible for the assessment and coordination of services required to meet the medical and behavioral healthcare needs of identified patients

  • Develops individualized care plans/goals with patient based on identified needs and required treatments recommended by healthcare providers

  • Based on assessment, monitors goals, interventions and progress of individual patients, determines appropriate frequency and type of contacts to meet care plan

  • Provide leadership and direction to ensure appropriate follow-through, reporting and documentation.

  • Educate patients about their health conditions, medications and medication side effects as needed.

  • Serve as an advocate and liaison between patients, their families, physician, mental health professional and facilities/agencies

  • Research community resources, trains behavioral routines and enables patients to be proactive in their own healthcare

  • Assemble information concerning patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialist.

  • Build relationships with patients by asking appropriate questions in an empathic manner and using good listening skills

  • Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).

  • Maintain accurate and thorough documentation of case management activities on the patient’s medical record.

  • Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health

  • Maintain professional and technical knowledge by attending educational workshops or reviewing professional publications to stay informed of eligibility and benefit changes to insurance programs

  • Maintain organizational and patient confidentiality at all times

  • Maintain a limited teaching load

  • Define and develop Project services with staff input

  • Monitor delivery of Project services and modify, if necessary, to improve services for teachers and students

  • Research, coordinate and participate in ongoing in-service training and education for teachers that relates to school day physical education or health education services

  • Provide ongoing assessment and feedback to Health Education or Physical Education Specialists or Instructors on quality of instructional services


Required qualifications for this position include:

  • Bachelor's Degree in Social Services, Social Work, Health Education, or related field

  • As required or applicable to specific programs (Current CA RN licensure a plus, Certified Health Education Specialist, Health Education Specialist, Certified Enrollment Counselor, MSW, LCMSW) to be obtained prior to starting employment

  • Current/valid Drivers license

  • At least 2 years experience working with community groups, employer groups and grant funded projects in a collaborative setting.

  • Excellent interpersonal skills, including the ability to engage with individuals or groups the first time that verbal or in person communication occurs

  • Ability to adapt to different personality types

  • Ability to coordinate multiple and varied details, prioritize duties and document services provided

  • Excellent writing, public speaking, organizational and communication skills

  • Working knowledge of government programs (i.e. Medi-Cal, Cal Fresh, FPL, VFC, Healthy Start, etc.,) when applicable to program being supervised

  • Willingness to adjust schedule as needed to complete necessary tasks

  • Ability to work independently or with minimal supervision

Preferred qualifications for this position include:

  • At least 1 year in experience in a supervisory role

  • Bilingual skills (English/ Spanish)
