Providence is calling a Advanced Practice Pharmacist (Full Time / Day Shift) to Providence Saint John's Medical Foundation in Santa Monica, CA.

We are seeking a Advanced Practice Pharmacist who will provide expertise and vision to Saint John's Physician Partners leadership in regards to clinical and fiscal pharmacotherapy issues. The Advanced Practice Pharmacist will work collaboratively with primary care physicians, specialists, staff and leadership to develop programs that improve patient outcomes through appropriate medication management. Develops and implements methods to measure the results of these programs to justify expansion and drive quality improvement. Provides coordinated, comprehensive, high-quality care to Saint John's Physician Partners patients. Responsible for the evaluation, interpretation and management of patient's medication therapies to improve the quality and outcomes of healthcare provided to our patients and control costs. Support clinicians and other health care providers through collaborative drug therapy management services, medication conversion programs and other functions to ensure that the use of pharmaceutical agents is rational and cost-effective. Interact with other departments and present reports and findings to Saint John's Physician Partners leadership as needed to perform the duties of this position.

  • Establish effective working relationships with Saint John's physicians and staff for the provision of high quality pharmaceutical care to ambulatory patients

  • Participate in Saint John’s Pharmacy and Therapeutics Committee (P&T); and other committees or meetings relevant to the functions of pharmacy services

  • Use, protect, and disclose Saint John’s patients protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards

  • Population-Health/Program Development:

  • Participate in the development of treatment algorithms, drug utilization management strategies, intervention techniques and policies and procedures needed to manage the Saint John’s pharmacy program

  • Be proactively and collaboratively develops and justify new and innovative advanced practice pharmacist services and models of care

  • Utilize pharmacy data, registries and other reports to develop and implement strategies to improve patient pharmacotherapy regimens

  • Implement and monitors drug conversion programs

  • Participate in the quality assurance and improvement process of the centralized refill service, disease state education clinics and anticoagulation service

  • Direct Patient Care:

  • Be responsible for providing collaborative drug therapy management (CDTM) in designated Saint John’s services in-person, telephonically, and by e-visits

  • Assess the patient, adjust medications, screen for drug interactions and adverse effects, provide disease/medication education, and explore opportunities to improve the effectiveness, appropriateness, safety and economic impact

  • Evaluate medications that are high-cost, high-risk or high-volume and implements strategies to decrease the total cost of health care while maintaining quality

  • Integrate pharmacist-led medication therapy management into existing services

  • Provide comprehensive medication review services and implements changes or makes recommendations to health care providers based on evidence based medicine and national guidelines to improve the quality and outcomes to our patients

  • Fully document all aspects of patient care, and completes all required paperwork for billing and administrative purposes

  • Exercise judgment on conditions requiring consultation, referral or evaluation by patient’s PCP or other health care professional

  • Educator/Preceptor:

  • Provide education to Saint John’s providers, staff and patients in written and verbal formats that include formal and informal presentations

  • Assist in clinical instruction and supervision of medical assistants, certified pharmacy technicians, pharmacy students and others as required

  • Participate in collaborative disease state education programs to improve the quality and outcomes to our patients

  • Educate patients and/or families about preventative care, medical issues and use of prescribed medications

  • Keep up to date on relevant medical research, technology and related issues by attending continuing education courses and professional meetings, reading journals, etc.

  • Serve as a drug information resource to Saint John's Medical Foundation

  • Participate in company-wide in-services and clinician presentations including CME supported activities

Qualifications:

Required qualifications for this position include:

  • PharmDDoctorate of Pharmacy from a college or school of pharmacy accredited by ACPE

  • ASHP-Accredited PGY-1 Residency (General Practice, Ambulatory Care, Community Pharmacy, or managed care ) or 2 years of pharmacy practice experience in disease state management in primary care clinics (diabetes focus preferred)

  • 3 years pharmacy practice experience in disease state management in primary care clinics (diabetes focuse preferred); managed care experience (managing medication prior authorizations and pharmacist-run medication refill program)

  • Experience with electronic medical records and electronic prescribing

  • Ability to communicate well verbally and in writing

  • Knowledge of all Microsoft software

  • Knowledge and expertise in clinical pharmacology and disease state management using most up-to-date evidence-based practice

  • Ability to educate and persuade patients and/or family members to alter behavior in accordance with program goals and objectives

  • Excellent organizational, interpersonal and decision-making skills.

  • Ability to work independently and manage multiple, concurrent projects for completion in a thorough and timely manner

  • Ability to analyze data and formulate sound conclusions and recommendations

  • General knowledge and understanding of pharmacoeconomics,

  • General knowledge and understanding of Accountable Care Organizations, MediCare PartD, HEDIS, CMS Stars, and Value-Based Purchasing, Commercial and managed care insurance contracts.

  • General knowledge of California Board of Pharmacy regulations and policies.

  • General knowledge of the CURES and California laws for controlled substances.

  • Current valid California Pharmacist license.

  • Current American Heart Association or BLS for Healthcare Providers

Preferred qualifications for this position include:

  • PGY-2 Residency (Ambulatory Care, Pharmacoeconomics, Managed Care)

  • Certification in Pharmacotherapy, Ambulatory Care, Diabetes or Geriatrics
