Providence is calling a Peer Support Specialist - Behavioral Health Operations (1.0 FTE, Days) to Providence Saint Vincent Medical Center in Portland, OR. This position will typically work Tuesday - Saturday, 9am - 5:30pm.

We are seeking a Peer Support Specialist who will be an essential part of the Behavioral Health team. Provides telephonic and in person support and assistance with connection to outpatient behavioral health, substance use and other community services to patients who have recently discharged from the hospital. The peer support specialist may work remotely, in the hospital, clinic, in the community, the patient's home and/or telephonically or wherever the patient will need support. Connects people served with available community services while promoting the use of natural supports and existing community relationships.
In this position you will have the following responsibilities:
Conduct in person and telephonic outreach to assigned patients, within hospitals, clinics and the community
Collaborate with patient's care team
Identify patient risk of harm to self and/or others, identifies patient's stage of change and implements effective risk reduction, including referring patient to appropriate level of care
Understand payer constraints relating to appropriate community referral resources
Ensure compliance guidelines are followed for all Federal, State and other regulations, which include, but are not limited to: CMS, The Joint Commission (TJC), EMTALA, COBRA, and HIPAA
Escalate problematic or high-risk interactions to manager for assistance
Maintain appropriate boundaries with patients, family/support and others involved
Required qualifications for this position includes:
High School Diploma -OR- GED -OR- equivalent experience and education
Current THW certification in the state of Oregon as a peer in ONE of the following capacities required:
Peer support specialist
Peer wellness specialist
Family support specialist
Family wellness specialist
Youth support or wellness specialist
Maintenance of certification through continuing education hours as outlined by the state of Oregon
Valid current drivers license
American Heart Association (AHA) Basic Life Support for Health Care Provider completion required, or obtained within one month of hire
One (1) year of providing peer support services
Basic computer skills including keyboarding, use of mouse, ability to navigate websites and use common web browser
Preferred qualifications for this position include:
Experience working with patients with chronic, acute and contingent crisis
Knowledge of Medication Assisted Treatment
