Providence is calling a Community Resource, Outreach Specialist (0.8 FTE, Days) to the Swindells Center at the Providence Child Center in Portland, OR
The Swindells Resource Center of Providence supports parents and caregivers of children who experience special health care needs.
We are seeking a Community Resource, Outreach Specialist that will support children and families, connecting them directly to resources and education available in the community. This position will be based at Swindells Resource Center, west, with some travel to our east site at Providence Child Center. Job responsibilities include responding to requests for child and family support, providing resources and education regarding community services, referring to community agencies, and promoting The Swindells Center’s and partnering agency events in the community. This role works collaboratively with other Swindells Center staff to manage a comprehensive library, speaker events, and other duties as assigned.
In this position you will have the following responsibilities:
Respond to requests for information related to disabilities from families, caregivers and service providers
Assist families, caregivers, and service providers in locating disability specific resource materials or information within the Swindells Center library, the community, or on-line
Refer families to community resources that can meet the identified needs of their child
Maintain knowledge of and connection with state and community based agencies serving children and youth with special needs, including eligibility, current contacts, waiting lists, and financial requirements in order to provide up to date resources to families
Provide social service and medical system support and coordination to families with a new diagnosis
Work with medical providers to include pediatric subspecialists, therapists and social workers, to help clarify diagnosis and resource needs of families
Assist parents in voicing their questions
Offer on-going 1:1 emotional and resource support to families
Plan & coordinates disability-related training events for parents, caregivers, and professionals including but not limited to identification of trainers, development of materials, agendas, planning and evaluation of programs
Participate in conferences and community and information fairs to disseminate information regarding Swindells Center services
Promote Swindells Center and partnering agency events in the community
Manage the library and speaker events
Establish collaborative working relationships with other community agencies
Complete all required paperwork and data entry on a timely basis
Participate in training opportunities as identified
Required qualifications for this position includes:
Bachelor's Degree -OR- equivalent education/experience
Valid drivers license
One (1) year of experience delivering services to families with children with special needs
