Providence is calling a Clinical Pharmacist Anticoagulation Specialist - Anticoagulation Clinic (1.0 FTE, Days) to Providence Portland Medical Center in Portland, OR
We are seeking a Clinical Pharmacist Anticoagulation Specialist - Anticoagulation Clinic that will collaborate with providers and caregivers to provide coordinated, comprehensive, high-quality care to patients referred to the PH&S Anticoagulation and Pharmacotherapy Services for focused drug therapy management of acute and chronic disorders. The Clinical Pharmacy Specialist is responsible for providing collaborative drug therapy management (CDTM) services in clinic, telephonically, by e-visits, and possibly telehealth. Upon referral, the Clinical Pharmacy Specialist will assess the patient, initiate and adjust medications, screen for and minimize drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify &/or decrease the cost of the medication regimen in collaboration with the referring provider (PCP)/prescriber, patient/their caregiver and other Providence team members. They will also educate team members to continually advance the service level of practice.
In this position you will have the following responsibilities:
Establish effective working relationships with providers and caregivers for high quality pharmaceutical care to ambulatory patients
Provide Collaborative Drug/Disease Therapy Management (CDTM) for patients who are referred as part of team-based care protocol according to the CDTM guidelines
Communicate, collaborate and provide warm-hand-offs to other clinical pharmacy specialists and caregivers in the hospital, clinic-based medical home, Elder at Home, or during other transitions of care
Efficiently and effectively complete work in support of consults, new patient onboarding, and other services and responsibilities
Required qualifications for this position includes:
Graduate from an accredited school of pharmacy
Coursework/Training: Post-graduate year residency (PGY1) -OR- three (3) years experience in area of specialization (ambulatory care with direct, collaborative patient care) in lieu of residency
Oregon State licensure as a Pharmacist or ability to obtain license prior to employment
Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacist (BCACP), or other Board of Pharmacy Specialties certification in area of specialization prior to or within two (2) years of hire as appropriate.
Basic Life Support (BLS) within six (6) months of hire
Hands-on experience in relevant patient assessment, medication adjustment, monitoring, and all other aspects of patient care
Experience in writing, including educational pieces for patients and healthcare providers
Experience precepting students and/or residents
Preferred qualification for this position include:
- Coursework/Training: PGY-2 residency in ambulatory care
