St. Joseph Health / Covenant Health Social Worker - Sonoma Hospice Direct in Petaluma, California

We are looking for a Social Worker (MSW) for the HCN Sonoma Hospice Direct Department at St Joseph Home Care Network.

Location: Petaluma, CA

Work Schedule: On call/Per diem

Shift: 8 hour, Days

Job Summary:

The Social Workers (MSW) evaluates and assesses patient needs for social service assistance, planning and coordination of clinical programs and acts as a liaison between the patient, hospital, Agency, Physician, Insurance Plans and community with in the St. Joseph Home Care Service Lines. Embraces and utilizes evidence based assessment and patient education tools. Understands and promotes the St. Joseph Home Health Social Service Program.

Essential Functions:

  • Documents the full assessment and create a plan of care with accurate orders, diagnosis and projection of visits for all disciplines necessary to support each patient in EHR.

  • Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.

  • Documentation completed and synchronized/submitted by 10:00 am the following day.

  • Documents patient status in an accurate, timely and concise manner by use of EHR in patient home at the time of visit.

  • Acceptance and response to referrals from interdisciplinary health care team regarding clients who may require social services assistance.

  • Interviews patients and families in order to assess psychological, social, emotional and economic needs.

  • Plans and conducts short-term goal oriented psychotherapy for patients and families.

  • Provides supportive and/or bereavement counseling to patients and their families to cope with illnesses and/or death.

  • Demonstrates self-motivation and initiative by developing and maintaining current information on community resources through attendance of continuing education and professional associations.

  • Completes documentation and paperwork as required by Network policies; following the physician's Plan of Care for Medical social Services.

  • Makes visits as scheduled and participates in on-call rotation as required.

  • Actively integrates Social work skills with quality improvement initiatives and agency programs related to patient safety and improved outcomes.

  • Participates in Geriatric Care Management program development and is an active member of Coalition Team.

  • Annually completes compliance education and understands applicable policies.

Skills :

  • Effective communication skills (verbal and written).

  • Good understanding of computer software and can easily adapt to computer programs.

  • Organizational skills.

  • Ability to follow Agency's policies and applicable laws.

  • Ability to respond to common inquiries or complaints, regulatory agencies or members of the business community.

  • Flexibility to adapt to new and changing situations.

  • Demonstrates customer service skills.

Minimum Position Qualifications:

Education : Master's Degree in Social Work.

Experience : 1 year of Social work experience.

Licenses/Certifications:

  • Basic Life Support certification.

  • California driver's license.

  • Current auto liability insurance.

Preferred Position Qualifications:

Experience : Experience in home health care setting.
