Location: Petaluma, CA

Work Schedule: On call/Per diem

Shift: 8 Hour Days (Prefer candidates with availability to work occasional weekends/holidays)

Job Summary:

Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.


  • Skill in family counseling, psychosocial assessment and utilizing community and health care resources.

  • Skill in teamwork and communication.

  • Knowledgeable about different developmental needs of children and teens. (Hospice Only)

  • Knowledgeable of psychosocial issues and challenges confronting geriatric clients and patients.

Minimum Position Qualifications:

Education: Master's Degree in Social Work, Psychology or Counseling or related field.

Experience: 1 year experience in a health care setting or related field.

Licenses and Certifications:

  • California driver's license.

  • Current auto liability insurance.

Preferred Position Qualifications:


  • Graduate from an accredited school

  • Coursework/Training - Training in family counseling and case management.

Licenses and Certifications: LCSW
