We are looking for a Nurse Navigator for the Cancer Center Admin Department at St. Joseph Hospital Orange.
Location: Orange, CA
Work Schedule: Full Time - 80 Biweekly Hours
Shift: 8-hour, Days
Under the direction of the Director of Nursing for Cancer Services and the designated medical staff program director, the nurse navigator coordinates the patient care, research components and program outreach for the patient diagnosed with cancer. This includes monitoring patients through screening/diagnostic evaluation, assisting patients with navigation through healthcare systems, provision of education and emotional support, identification and development of programs and services to enhance patient care and support for program outreach to referring physicians and the community.
Serves as a knowledgeable and reliable source of information for patients regarding diagnostic testing, next steps, and treatment options. Assists patients from screening, to point of diagnosis and throughout treatment and survivorship to anticipate patient concerns, negotiate and coordinate appointments, obtain second opinions, and ensure patients have access to clinical and educational materials to meet their needs.
Serves as a reliable source of information and support for patients regarding supportive/holistic care (including the use of the distress screening scale which includes measurements of psychosocial, emotional, financial and spiritual support). Assists patients with referrals to other members of the SJO Cancer Team including but not limited to, financial counseling, clinical social work, dietitian and community support agencies.
The Nurse Navigator serves as a pivotal link for cancer research by demonstrating familiarity with available program-specific clinical trials, the associated eligibility and exclusion criteria, and the informed consent content and process. He/She will routinely screen potential participants for available studies including review of malignant pathologies, new physician consults and other sources of newly diagnosed patients or those with recurrent disease to determine eligibility status. He/She will assist the Research Department with communication and coordination of required research information, as needed and provide routine updates on patient status with the managing physician and the Cancer Research Department.
The Nurse Navigator is responsible to facilitate, organize and coordinate clinical activities associated with the assigned site-specific cancer program. Activities include but are not limited to: clinical activities, program development and support, coordinating meetings (agenda and minutes), developing new services, supports and maintains the case conference activities, cancer screening programs at SJO, as appropriate. Coordinates the cancer screening programs at SJO, as appropriate. Develops and maintains strong relationships with cancer physicians, office managers and diagnostic services utilized in the screening, treatment and marketing of programs and services. Works with the program Medical directors and the clinical and administrative team to identify and resolve service issues.
Provides clinical information and support for Marketing, Business Development, Community and Employer Relations outreach activities that increase the visibility of SJO and its perceived value within the Orange County community.
Communicates routinely with cancer leadership and reports on activities and outcomes proactively. Develops, maintains and reports metrics for navigation services and program. Tracks, monitors and helps prioritize allocated program funds as appropriate. Delegates appropriate tasks and projects to the resource team in an efficient and effective manner with clear instructions, timelines and prioritization. Is sensitive to overall department demands in task delegation recognizing the potential for priority conflicts and facilitates resolution in a team-oriented manner.
Evaluates appropriate literature and peer review articles for development/implementation of nursing/supportive care research and/or performance improvement initiatives. Plans, implements and follows-up on all phases of clinical activities of relevant nationally recognized guidelines including the NCCN guidelines.
Accountable for managing, and evaluating educational materials for the patients and their families. Educational materials used have appropriate clinical content and information that is determined to be appropriate for the educational level and language of the populations served.
Supports programmatic activities for quality, resource utilization, outcomes and clinical practice.
Knowledge of the oncology disease process, treatment protocols for chemotherapy and radiation therapy, treatment side effects and outcomes for the process.
Ability to support patients and their families in the end-of life process and provide supportive care.
Can collect, organize and analyze data, synthesizing it into understandable information.
Possesses the skills to effectively communicate, problem-solve, and maintain productive and effective interpersonal relationships while effectively prioritizing.
Flexibility to adapt to new and changing situations.
Ability to think critically.
Able to give constructive feedback, supervises and teams with others, teaches staff and patients.
Possesses basic computer skills.
Ability to work independently, triage patients and keep physicians informed of patient status.
Work with outside agencies on a routine basis, maintaining positive working relationships as well as assuring the financial viability of the hospital.
Knowledgeable with Microsoft Office Suite (Word, Excel, Access, etc.).
Minimum Position Requirements:
Education: Bachelor's Degree in Nursing.
Experience: 5 years of experience as a Registered Nurse in a health care setting, or 3 years experience in oncology nursing
Additional Skills: Bilingual in Vietnamese
Current California RN license.
Current American Heart Association Basic Life Support (BLS) certification.
One of the following certifications (or successful completion within 1 year of date of hire):
Oncology Certified Nurse (OCN), or
Advanced Oncology Certified Nurse (AOCN), or
Advanced Oncology Certified Nurse Practitioner (AOCNP), or
Advanced Oncology Certified Clinical Nurse Specialist (AOCNS), or
Certified Breast Care Nurse (CBCN).
Preferred Position Qualifications:
Education: Master's Degree in Nursing.
Experience: Community education experience.
