Providence Care Management Social Worker - Utilization Review in Olympia, Washington
Providence is calling a Care Management Social Worker (1.0 FTE, Day Shift) to work for the Utilization Review Department at Providence St. Peter Hospital in Olympia, WA.
The Care Management MSW is under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre-admission to immediate post-discharge. Acts in a non-caregiver capacity by facilitating coordination and communication between all member of the health care team, patient and family in the decision making process to minimize fragmentation of the health care delivery system. The Master of Social Work functions as part of a professional care coordination team of nurses and social workers and other clinicians to provide skilled psychosocial services to patients and their families. The MSW is a resource to clinical staff to assist them in evaluating patient needs for psychosocial and emotional support and to patients and families for dealing with crises, transitions and continuity of care.
In this position you will:
Assesses patients for care management services after a referral has been made based on the Nursing Admission screening for potential patient.
Appropriately identifies actual and potential care issues within scope of practice.
Develops and manages a coordinated plan of care to meet care managed patient and/or family needs within scope of practice.
Recognizes circumstances requiring nursing intervention.
Monitors the patient/family progress toward goal achievement of optimizing functional status.
Required qualifications for this position include:
Master's Degree in Social Work or Related Field.
Three (3) years of experience in social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination.
Ability to work independently and take a leadership role to accomplish objectives.
