Providence is calling a Case Management Associate (LVN) (Per Diem/Day Shift) to Providence Holy Cross Medical Center in Mission Hills, CA.

We are seeking a Case Management Associate (LVN) who will be responsible for assisting the case managers to identify the discharge needs of the patient and family and help facilitate a smooth and safe transition from the hospital. The position is responsible for assisting the case managers with all the discharge planning needs of patients that are assigned and utilizes the Case Managers for situations that are out of his/her scope of practice.

In this position you will:

• Demonstrate Providence Health System San Fernando Valley Service Area's core values of respect, justice, compassion, stewardship, and excellence to customers, employees, and visitors; and provides quality service in the performance of work assignments and duties (3, 4)

• Maintain established departmental policies and procedures, objectives, improving organization performance program, and safety standards (4)

• Interact with medical support staff and physicians assigned to individual patients to facilitate the discharge process (i.e., identify what needs the patient or family may have) (1)

• Oversee, with the instructions of a Case Manager, the appropriateness of stay in the hospital, both acute and skilled (1)

• Meet with multidisciplinary groups to discuss discharge plans on a daily basis (1)

• Participate in administrative staff meetings on a monthly basis and attends others as appropriate to enhance professional growth (4)

• Identify patient/discharge needs to assure that both receive support with resources and placement assistance (1)

• Assure appropriate documentation is completed on patient records as identified in department policies (1, 2, 4)

• Facilitate the transition home by ordering equipment, arranging and referring home health visits, contacting the HHS prior to discharge, and initiating a follow up telephone call after discharge (1, 3)

• Perform other duties as assigned or requested

• 4 Failure to perform this function may have serious consequences

Qualifications:

Required qualifications for this position include:

• License Vocational Nurse ( LVN )

• Experience in a health care related position

• 1 year experience in acute hospital case management & discharge planning.

• Knowledge of medical terminology

• Knowledge of medical equipment

• Medicare/MediCal insurance guidelines for acute and skilled stay

• Strong communication skills

Preferred qualifications for this position include:

• Associate's Degree

• Home Health, Skilled Nursing facility discharge planning experience

• Medical Group/IPA case management experience
