Providence is calling a Manager Home Health Clinical Services - Home Health Services (1.0 FTE, Days) to Providence Medford Medical Center in Medford, OR
We are seeking a Manager Home Health Clinical Services - Home Health Services that will provide leadership to coordinate and administer efficient and effective delivery of interdisciplinary care to the patient in the home. This includes responsibilities in ongoing operations, and fiscal accountability for select components of department's operating budget. In conjunction with supervisors develops and implements all policies and procedures for Home Health, both department specific and those that apply to Home Services. Facilitates collaborative working relationships within Home Health and programs within Home Services. May represent Home Health to all publics, internal and external. Upholds the standards of confidentiality, attendance and punctuality.
In this position you will have the following responsibilities:
Directly involved with recommendations for budget development and monthly monitoring of operations utilization and expenses including reviewing staff productivity standards, and payroll reports, recommending and implementing ways to decrease overtime. Includes oversight of cell phone plan administration
Develop/Maintain Clinical Staffing Plan. Reviews recommended new positions of FTE changes to respond to increased patient care visits and processes posting upon approval
Act as HR liaison for Home Health staff issues
Be responsible for interviewing qualified supervisory and other staff and hiring
Counsel employees, oversees improvement plans as indicated and terminates, if necessary
Be responsible for outside staffing contracts
Participate with the Director of Clinical Operations, Home Health management team and other key Home Services personnel in developing, evaluating and revising home care policies, procedures, standards, competencies, quality and risk management, and objectives to ensure they cover clinical operations and processes and they meet industry standards and regulatory requirements
Be responsible for monitoring the compliance to meeting policies, procedures and standards to assure quality and consistency of services among the various sites and teams and to participate in the provision of in-service education as needs are identified to obtain desired results.
Participate in rotation of after hours Manager Call. Assignment is for 1 week at a time from 4:30 pm – 8:00 am Monday-Friday and 24 hours/day on Saturday/Sunday
Designate a qualified person to act in his/her absence
Oversight of communication, coordination and problem solving with physicians, patients, families and staff and payors
Serve as a liaison to hospital, discharge coordinators and community agencies as appropriate to ensure continuity of care for patients transitioning between providers
Participate in development as a member of the department management team, on CQI teams, and as appropriate representatives on HSD committees, hospital committees, regional and/or community committees
Collaborate and coordinates with Director of Clinical Operations and the Home Services Quality Manager to develop, implement and monitor a quality management program for Home Health
Determine that quality outcome objectives are met and initiates appropriate changes when objectives are not met
Participate in QM, utilization and record review activities, recommending following-up plans to obtain desired results as appropriate
Be an active role in resolving customer concerns, especially when related to process
Develop and maintain processes to assure continuity and consistency of services, policies and systems among the Home Health sites through effective communication, both verbal and written
Serve as liaison between staff and supervisors at each site to keep them current and informed of changes to standards, policies, procedures, form utilization, etc.
Assure the coordination of scheduling activities of patient assignments between sites
Participate as a team leader during regulatory reviews, i.e., State, Federal, and JCAHO
Coordinate after-hours services and staff to promote quality care and access for all patients, families and physicians
Act as lead for special projects as assigned by Director of Clinical Services
Provide direct coverage for the Clinical Supervisor, as needed
Plan and implement, with Home Services Clinical Operations Director and Home and Community Services Marketing Manager, marketing programs specific to all areas under her charge as appropriate
Inform Home Services Clinical Operations Director of ongoing operations, including conformity to policies, budget and volume trends/changes, and issues of concern
Maintain and enhance personal expertise through appropriate educational experiences in professional clinical and administration areas
Qualifications:
Required qualifications for this position includes:
Current Nursing License for the State of Oregon
Three (3) years of Management -OR- administration experience
Three (3) years of Nursing experience
Preferred qualifications for this position includes:
Bachelor's Degree in Nursing
Master's Degree in Nursing -OR- related Healthcare field
