Covenant Health is seeking for Licensed Vocational Nurse for the Hospice – Lubbock at Hospice of Lubbock Inc.!
Assists Admissions Coordinator with the intake of referrals, obtaining patient information and physician orders, coordinating services with social worker and performing admissions of appropriate patients. Ensures that patient care records are accurate and complete and assists with the performance improvement program of Hospice. Requires minimal supervision.
Process referrals for hospice patients. Plans for admission of patients to hospice in coordination with Admissions RN and patient/family and/or facility.
Assumes initial responsibility for assessing patient/family needs and consulting with attending physician, Hospice Medical Director and other hospice staff members. Determines patient's appropriateness for hospice services consistent with Hospice's policies and admission criteria and in conjunction with patient’s physician.
Obtains required documentation for the admission of patients desiring hospice services; secures facility plan of care and physician orders for admission (including Medication Profile) and obtains data on physical, psychosocial, social and spiritual factors that may influence patient/family health status. Completes initial entry into EMR.
Facilitates transition of patient/family to primary nurse and other members of the IDT as indicated. Seeks direction from Clinical Manager or Alternate Nursing Supervisor for conflict resolution or difficult case assignment.
Initiates phone contact to make appointment with patient/family to provide information on hospice services and/or assess patient’s status.
Assist in the coordination of the patient’s discharge from the hospital and admission to hospice either at home, SNF or Assisted Living including transportation, equipment, provision of care by family/hired help, medications and supplies as needed
Participates in departmental performance improvement and quality management functions.
Maintains a committed and cooperative attitude with staffing, promoting teamwork, effectiveness and efficiency.
Minimum Position Qualifications:
- Licensed Vocational Nurse
One year nursing experience, preferred.
Experience in working with terminally ill patients, preferred
LVN with current Texas license.
BLS (AHA) certified with thirty (30) days of employment.
Current driver’s license and proof of current auto liability insurance.
Preferred Position Qualifications:
One or more years’ experience in hospice.
Certified Hospice and Palliative Nurse (CHPN).
