Providence Health & Services Physical Therapy Assistant - Home Health Sound in Lacey, Washington

Description:

Providence is calling a Physical Therapy Assistant (PTA) (Home Health, Day Shift, 1.0 FTE) to Providence SoundHomeCare and Hospice in Lacey, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.

In this position you will:

  • PT care needs are assessed as needed for PT patients

  • After a referral, an accurate history and physical exam are performed.

  • Documents assessment data within 24 hours of home visit.

  • Describes assessment and accurately interprets data holistically (psychological, physiological, cognitive and medication regime).

  • Observes reimbursement eligibility criteria in implementing services for patients.

  • Assists in obtaining information to determine alternative payment sources.

  • Communicates with authorization case management to determine eligibility for home care.

  • Determines priorities of client care based on client's needs and available resources

  • Manages time effectively.

  • Prioritizes client's needs effectively

  • Completes Agency documentation such as daily schedules, time sheets, mileage reports per SHC guidelines, unusual occurrence, employee injury and paid time off requests

Qualifications:

Required qualifications for this position include:

  • Associate's Degree in Physical Therapy Assistance or or a graduate of an approved school of physical therapy assistant who is eligible for licensure.

  • Washington State Physical Therapy Assistant License

  • Healthcare Provider BLS/CPR certification required.

  • Valid Washington State driver’s license.

  • Insured transportation.

  • 1 year Experience in an acute care facility or a graduate of an approved school of physical therapy assistant who is eligible for licensure.
