Job Summary:

Under the direction of the Occupational Therapist, provides treatment to patients requiring occupational therapy services within the homecare setting. Provides professional occupational therapy visits in the homecare stetting in accordance with physician's orders through direct care and multidisciplinary team coordination. Participates in the utilization review, performance improvement and interdisciplinary conference of the organization. This position reports to Clinical Manager and is supervised by the Occupational Therapist.

Essential Functions:

  • Documents the full assessment and create a plan of care with accurate orders, diagnosis and projection of visits for all disciplines necessary to support each patient in EHR.

  • Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.

  • Documentation completed and synchronized/submitted by 10:00 am the following day.

  • Documents patient status in an accurate, timely and concise manner by use of EHR in patient home at the time of visit.

  • Performs occupational therapy services under the supervision of the occupational therapist.

  • Provides occupational therapy services to the patient in the home setting based on the plan of care, authorizations and physician orders following ordered frequencies within the Medicare week; adheres to agency policies and procedures for occupational therapy, infection control, bio-hazardous waste management, quality improvement activities, safety and risk management practices. Assures continuation of therapeutic plan by designing home exercise programs, instructing patients, families and care givers in home exercise programs; recommending and providing assistive equipment, as needed.

  • Reports changes in the patient’s condition to the occupational therapist.

  • Teaches the patient/family regarding active and passive exercises and the patient’s total occupational therapy program.

  • Accurately and thoroughly documents patient care observations, interventions, and evaluations; prepares and submits completed documentation of the home care visit within the established time parameters in agency policy.

  • Supports coordination of care activities among all disciplines on the care plan by participating actively in case conferences, identifying additional services needed and discharge planning with multidisciplinary team and other resources to achieve patient outcomes.

  • Makes ongoing therapy re-assessments, evaluates patient progress, updates the plan of care based on findings, and reports significant changes in the patient’s condition to the physician, appropriate team members and/or Clinical Manager.

  • Maintains patient schedules based on orders and authorizations within the Medicare week; submits caregiver schedules timely; and accurately; communicates scheduling changes daily; meets productivity standards demonstrating effective time management skills.

Minimum Position Requirements:

Education: Education to meet certification, license or registration requirement.

Licenses/Certifications:

  • Current Occupational Therapy Assistant License.

  • Basic Life Support certification.

  • California driver's license.

  • Proof of current auto liability insurance.

Preferred Position Qualifications:

Experience: 1 year in home health as an Occupational Therapy Assistant.
