St. Joseph Health / Covenant Health Nursing Assistant I - Subacute in Healdsburg, California
We are looking for a Nursing Assistant I for the Subacute HH Department at NorCal HealthConnect.
Location: Healdsburg, CA
Work Schedule: Full Time - 72 Biweekly Hours
Shift: 12-hour, Nights
Assists patient with the activities of daily living; including oral care, ambulation, personal hygiene, assisting with transfers to and out of bed/chair. Under the supervision of a registered nurse, provides basic nursing care to patients and assists in the maintenance of a safe, clean environment; all while acting as a patient advocate. As well as the ability to take vital signs properly, both manually and mechanically.
Documents correctly using standard forms and accepted format and abbreviations, including patient interactions and accurate assessment of emotional and physical status.
Demonstrates effective knowledge of responsibility in cardiac/respiratory arrest; serves on Code Blue and Code White Teams.
Understands and assists in the appropriate use of restraints and assistive devices for patient treatment and safety.
Effectively assesses patients for fall risks and initiates appropriate action, utilizing preventative devices safely and appropriately.
Understands patient’s rights regarding care, confidentiality, and release of information.
Encourages the promotion of patient’s independence in decision-making while maintaining safety.
Assists Care Team with patient care requiring proficient knowledge of the following vital signs, including orthostatic blood pressure, pulse, respiration:
Nutrition and hydration requirements.
Patient hygiene including proper attire, oral hygiene, skin care, elimination requirements and patient positioning to prevent injury.
Specimen collection and handling.
Demonstrates the ability to accurately record intake and output, including percent of dietary intake including carbohydrate counts on Diabetes patients.
Demonstrates knowledge of the care of patients with IV’s, Foley catheters, NG/gastric tubes and drains.
Participates with nursing team in hourly rounds and huddles.
Participates with Lead Nurse in CNA rounds.
Reports to Lead RN throughout the shift when necessary about patients and assignments.
Attends 90% of all CNA classes held at HDH. Exceptions may apply if enrolled in higher education.
Performs other duties as assigned to support the department’s objectives.
Able to handle a busy floor and multi-task.
Ability to keep good communication and a positive attitude with co-workers and patients.
Effective communication skills.
Ability to read and write, understand and follow instructions.
Minimum Position Requirements:
Education: H.S. Diploma or GED
Current C.N.A. License.
Current Basic Life Support (BLS) with Skills Check-Off
Preferred Position Qualifications:
Experience in an acute care setting.
1 year Nursing assistant experience in acute or long-term setting.
