We are looking for a Nursing Assistant for Healdsburg Subacute for Nor Cal Health Connect.

Location: Healdsburg, CA

Schedule: Full Time - 72 biweekly hours

Shift: 12-hour Nights

Job Summary:

Under the supervision of an RN or LVN, the CNA/Care Partner will provide direct patient care within

the scope of practice.

Essential Functions:

  • Responds to the clinical and service needs of patients and families in a proactive way.

  • Demonstrates competency when performing patient care procedures within scope of practice.

  • Maintains safe working environment by adhering to all hospital and department policies and

  • procedures. Identifies and reports unsafe procedures.

  • Documents data collected and care provided within scope of practice.

  • Provides patient comfort and privacy during all interactions.

  • Reports patient/visitor concerns, complaints and information to nursing staff.

  • Demonstrates accountability for apprising self of hospital and department

  • communications/information.

  • Demonstrates commitment to department objectives and standards.

    Skills:

  • Ability to communicate effectively (both verbal and written).

  • Ability to interact with patient, co-workers, and physicians in a courteous, service oriented

  • manner.

  • Demonstrates an attitude of cooperation and flexibility by responding to varying and

  • unpredictable situations.

  • Demonstrates ability to walk and stand for prolonged periods of time. Ability to transport and

  • lift patients using hospital approved lifting devices or techniques.

  • Ekg.

  • Assemble oxygen equipment.

  • Blood glucose monitoring (acute only).

  • Attend four (4) hours of Care Partner training offered by St. Joseph Health System – Greater

  • Sonoma County.

  • All ratings on performance evaluation are on target (OT) or above.

    Position Requirements:

    License / Certification:

  • Current CNA Certificate required for SNF departments/preferred for other departments

  • CPR Certification within 60 days of hire.

    Experience:

  • Previous healthcare experience in training program or in work setting.

    Preferred Position Qualifications:

  • Medical terminology

  • Computer skills
