St. Joseph Health / Covenant Health Social Worker - Community Care Navigator (Grant Funded) in Fullerton, California

We are looking for a Social Worker with Community Care Navigator Department at St. Jude Medical Center.

Location: Fullerton, CA

Schedule: Part-time 64 Biweekly Hour

Shift: 8 Hour Day Shift

Job Summary:

This position is grant funded for the period of 1 year.

Provide support to and builds a relationship with high risk, chemical dependency and mental health patients who are frequent emergency department admissions and /or who discharged from the hospital to ensure a successful transition from ED/ hospital to the community and to reduce re-admissions to the ED/hospital. Support includes ensuring patient has housing and appropriate mental health / chemical dependency resources, follow-up physician and ancillary appointments, arranges transportation to medical appointments if needed, and is receiving the equipment and services prescribed in the patients discharge plan. Ability to assess mental health and chemical dependency patients for appropriate resources, services and coordination of care. Serve as a link between organizations serving mental health and chemical dependency patients and the Medical Center to improve coordination of services. Supervise service learning and social work interns as necessary.


  • Must possess excellent written, verbal, listening and interpersonal communication skills

  • Demonstrates strong customer service orientation; Must demonstrate critical thinking skills

  • Computer literate and able to learn how to access Epic and Meditech

  • Strong understanding of needs of and resources for chemical dependency and mental health

  • Possess knowledge and ability to do client intake requiring both assessment of client history concerning both short-and long-term needs

  • Maintain working knowledge of and adherence to all agency policies and procedures.

Position Requirements:

Education: Bachelor’s degree in human services, social work, marriage and family therapy or related field.

Experience: 3 years’ experience working with the community related to vulnerable populations, mental health services and chemical dependency issues.

Preferred Position Requirements:

Education: MSW or MA degrees

License and Certification:

  • LCSW or MFT.

  • CADC or CATC
