Providence Cultural Health Navigator Clinical- 100% Remote in Everett, Washington
Providence is calling a Cultural Health Navigator Clinical (1.0 FTE, Days) to work in Administration at Providence Medical Group Northwest Washington in Everett, WA.
This position is 100% remote, but you must live in and be familiar with Snohomish County
The Cultural Health Navigator reports to the Manager of Population Health, and must be comfortable working with individuals who may have medical and/or behavioral health conditions and/or substance use disorder. The Cultural Health Navigator works directly with individuals and their families to identify and address barriers to health through screenings and assessments (i.e., gaps in care, food, housing, transportation, etc.). The Cultural Health Navigator will also be conducting home visits to individuals at their place of residence (i.e., homeless shelter, assisted living, skilled nursing facility) or in the community, and will serve as an ally and liaison for helping provide access to care. The Cultural Health Navigator provides navigation, mediation, advocacy, care coordination and referrals to case management and community-based social services, and works with individuals from our inpatient, ED, outpatient care settings and community organizations. The Cultural Health Navigator must represent the communities they serve and have familiarity with resources in those communities. Cultural Health Navigators must have knowledge and/or understanding of trauma informed care and the impact trauma has on health care delivery within our BIPOC communities as well as an understanding of cultural systemic racism and inequities that exist throughout our society.

Refers clients/patients to the appropriate programs in the community that meet the needs identified in the screenings.
Document all outreach events and direct services provided in the electronic medical record, including maintenance of logs and sign-in sheets.
Employee completes all tasks and duties accurately and timely within assigned shifts. Demonstrates initiative to perform job duties autonomously.
Attends Mandatory and other education offerings per department and organizational requirements. May assist with education within the department.

Required qualifications for this position include:
Clinical degree outside of the United State - Nursing, Medicine, Public Health
Bilingual Fluent in Spanish, Arabic, Mandarin/Chinese, Vietnamese, or Marshallese
1 year of related Community Health or Outreach experience
Valid Driver's License and Auto Insurance
CHW or Clinical Certification or Licensure. Received CHW Certification or clinical/medial certification or licensure from country of origin.
Preferred qualifications for this position include:
LPN OR Clinical Licensure in Nursing or Medicine
Associate's Degree in Nursing
Certified/Qualified Medical Interpreter - Bilingual in Spanish, Arabic, Mandarin/Chinese, Vietnamese, or Marshallese.
