Providence Health & Services Social Worker MSW in Cerritos, California
Providence is calling a Social Worker MSW (Full-Time/Day Shift) to Providence TrinityCare Hospice in Cerritos, CA.
We are seeking Social Worker MSW who will be a team member of the hospice Interdisciplinary Group (IDG). The Clinical Social Worker provides bio-psychosocial services to patients/families/caregivers which include assessment, care planning, intervention and evaluation. May provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.
Schedule : 8am - 5pm with occasional on-call weekend shifts
In this position you will:
Complete assessment of the psychosocial status of pediatric patients/families/caregivers related to the patient’s terminal illness and environment within 5 days of patient/family admission to hospice care, unless deferred by patient/family
Provide social work intervention according to standards of clinical practice
Assist patient/family through the grieving process
Facilitate problem-solving and decision making with the patient/family
Assess potential and actual crises and provides crisis intervention
Perform suicide/homicide assessments on high risk patients/families, providing appropriate follow-up
Organize and lead family conferences as needed
Explore patient/family financial problems and makes referrals to appropriate organizations, i.e., SSI, MediCal, Senior Citizens Tax Relief, Hospice Foundation
Assist family in finding personal care and/or domestic help for patients
Assist in placing patients in care facilities when care at home is no longer viable
Facilitate referrals and decision-making about advanced directives, POLST, DPOAs, care and other legal or personal business
Provide bereavement follow-up per Plan of Care
Develop patient/family Plan of Care with the hospice IDG
Identify psychosocial problems and opportunities and establishes plan for intervention
Submit documentation for the patient medical record within the timeframe set by the organization guidelines
Participate in orientation, education and development of team members as directed by supervisor
Participate in appropriate continuing education as needed, or requested by immediate supervisor
Participate in the Performance Improvement Program and participates in and adheres to the LCMHS/TrinityCare Hospice Corporate Integrity Plan
Comply with patient care requirements of all regulatory agencies and accrediting bodies, including working knowledge of policies and standards of practice
Required qualifications for this position include:
Master's Degree in Social work
Must be a valid licensed driver with an automobile that is in good working order and insured in accordance with state and/or LCMHS/TrinityCare Hospice requirements
1 year experience in health care
Understands hospice philosophy and issues of death and dying
Demonstrates good verbal and written communication skills and organization skills

Preferred qualifications for this position include:
Licensure (LCSW) to practice in the state of California
Experience in hospice care
Bilingual with Spanish skills
Demonstrates working knowledge of computer applications, i.e. Microsoft Office and e-mail
