Providence is calling a Speech and Language Pathologist (Per Diem/Day Shift ) to Providence Saint Joseph Home Health based in Burbank, CA.

We are seeking a Home Health Speech and Language Pathologist who will evaluate patients of all ages regarding the application of a wide variety of therapeutic techniques for rehabilitation of speech, language, swallowing, and oral-motor disorders. Demonstrate proficiency in delivering care to assigned age-specific patient population (e.g. infants, pediatrics, adolescents, adults, and geriatrics).
In this position you will:
Identify, assess, and provide treatment for communication, cognition, and swallowing function and their disorders as ordered by the attending physician
Assess and adapt the patient's environment with regard to privacy, safety, cultural and religious practices, age, physical and/or cognitive limitations, and language barriers
Demonstrate ability to perform patient and employee safety standards appropriate to the department’s scope of care. The speech pathologist demonstrate competencies in patient and employee safety, personal protective equipment, safe transfers, hazardous materials, disaster, evacuation, incident reports, infection control, and administrative policies and procedures. Demonstrate willingness to report medical/health care errors, opinions and needs as well as perceptions of risks to patients and makes suggestions for improving patient safety, and behavior management
Is responsible for completion of annual updates, staff competency assessment, continuing education requirements, CPR certification, continuing education, and licensure
Assist the physician in evaluating the level of function
Advise, consult and when appropriate, instruct the family and other agency personnel, in the patient’s therapy program
Identify patient/client needs and assists the patient’s physician with the development of a treatment plan to meet those needs
Observe, record and report information on the patient’s condition to the attending physician and in the patient’s health record
Evaluate the home environment and makes appropriate recommendations
Teach and supervise other health personnel when appropriate
Attend patient care staffing, conferences and other related meetings
Maintain established departmental policies and procedures, objectives, improving organization performance program, safety, environmental and infection control standards, per hospital, TJC, local, state, federal guidelines
Maintain professional affiliations and enhances professional growth and development to keep abreast of latest trends in rehabilitation services
Maintain timely departmental reports, records, and files as required, per agency, TJC, local, state, federal, ASHA guidelines
Assist with care and maintenance of department equipment and supplies
Make home visits
Qualifications:
Required qualifications for this position include:
Master's Degree in Speech Language Pathology or Communication Disorders
Current BLS for Healthcare Providers certification
Licensed/registered by the Speech-Language Pathology and Audiology Board of California
Current California Driver's License
Minimum 1-year experience in providing Speech and Language Services
Preferred qualifications for this position include:
