We are looking for a Speech and Language Pathologist with Rehab Outpatient Department at St. Jude Medical Center.
Location: Brea, CA
Schedule: Full-time 80 Biweekly Hour
Shift: 8 Hour Day Shift
Under the direct supervision of the Program Manager and consultative supervision of the Speech Pathology Practice Manager, this Speech Pathologist is responsible for providing quality care for patients receiving Speech Pathology services, including evaluation, treatment planning, goal setting, treatment implementation, and discharge planning to transition the patient toward maximum independence. The Speech Pathologist plans and provides patient, family, peer and community education, and coordinates services with other health care professionals including interaction with physicians responsible for the care of the patient. The Speech Pathologist documents according to program and professional standards. The Speech Pathologist follows an individualized performance development plan to maintain current and competent in critical areas and support program goals, hospital goals and objectives, hospital functions and philosophy.
Screens, accepts and responds to Referrals, makes referrals to appropriate resources and educates potential referral sources about the scope of services and the process of initiating services.
Performs appropriate Evaluations and on-going assessments in a timely fashion to obtain the data necessary for patients with various diagnoses on assigned program.
Establishes Intervention Plan and Goals that are clear, measurable, behavioral, functional, contextually relevant, and appropriate to the client’s needs, desires, and expected outcomes. Incorporates patient and family goals.
Implements treatment Intervention through specified purposeful activities and therapeutic methods that are meaningful to the patient. Modifies the process to reflect changes in client status, desires, and response to intervention.
Performs Discharge Planning to facilitate smooth transition to next level of care; assures patient status is maintained or enhanced through home program instruction, caregiver training, and referral to appropriate follow-up resources.
Completes all required Documentation according to program standards, reflecting patient-centered care, by relating interventions to established goals and justifying skilled need.
Meets program Productivity Standard while meeting outcome expectations. Participates in data collection and analysis.
Ability to perform at established guidelines for program specific requirements.
Master's Degree Graduate of an accredited Program in Communicative Disorders. Or equivalent educ/experience
Registration for the Clinical Fellowship Year. Or equivalent educ/experience
Experience: At least 300 clock hours in providing direct patient care. Internships are acceptable to meet this requirement.
License and Certification:
Current and valid license as a Speech Pathologist, issued by the State of California, or Registration with the State of California for completion of licensure requirement (RPE). During this time, a temporary license will be issued by the State.
Certification of completion of training in Cardiopulmonary Resuscitation (Class C)-BLS.
Preferred Position Requirements:
Experience in neuro-rehabilitation, dysphagia.
6 months Experience in any capacity in an acute care or rehab hospital.
