Providence Health & Services Social Worker - PEC in Anchorage, Alaska
Providence is calling a Social Worker (Nursing Administration, Day Shift, 1.0 FTE) to Providence Extended Care in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will have the following responsibilities:
Manage a caseload and maintain an organized, current case file system that reflects the psychosocial needs of the resident and the efforts made to meet those needs
Record social work interventions, observations, assessments, and other important data, in the electronic medical record, that reflects the psychosocial needs and etc.
Participate in MDS/CAA process
Provide Minimum Data Set information and Resident Assessment Protocol summaries for designated MDS sections
Honor established MDS/CAA timelines to meet regulatory requirements
Complete social histories for each resident at admission, as appropriate
Ensure that the residents comprehensive care plan and resident daily care plan include interventions that address social services needs.
Assist new residents and their families in adjusting to life in their new setting
Assess and identify the medically related psychosocial needs of residents and develop a plan of care to meet those needs
Develop at the time of admission and maintain an active discharge plan for each resident at the time of admission
Participate in multi-disciplinary care conferences and intra-agency meetings to coordinate treatment, needs assessment, and discharge planning, as appropriate
Arrange for and coordinate the discharge or transfer of appropriate residents to their homes or other community placements, and assure continuity of care
Meet the psychosocial needs of residents by providing short-term individual and/or family counseling, crisis intervention services, referral to community resources and pertinent information as needed or directed
Provide training, consultation, and assistance to facility personnel, as needed or requested
Maintain and enhance professional competence and personal growth through on-going education, in-service training, workshops and seminars
Accept and integrate social work supervision as arranged or provided by the facility, should the desire be to obtain LCSW
Adhere to the professional guidelines and principles of NASW standards
Maintain appropriate standards of dress, professional behavior, and ethical conduct when functioning as a representative of the facility
Implement care/services that recognize age/diversity specific needs/issues of customers served
Complete Competency Plan for assigned job and department
Required qualifications for this position includes:
Bachelor's Degree in Social Work
Alaska Licensed (or provisionally licensed) Baccalaureate Social Worker (LBSW) or Master of Social Work (LMSW)
Current BLS certification from American Heart Association upon hire or the ability to accomplish within 30 days of date of hire
Preferred qualifications for this position includes:
Master's Degree in Social Work
One (1) year of recent experience in a medical facility or long term care setting
About Providence Extended Care at the Cottages
Providence Extended Care at the Cottages is located on a campus that includes eight cottages and a Commons Building. The Cottages are home to 96 residents. The Commons Building includes the facility reception area, Resident Bank, Gift Shop, Rehabilitation Therapies, Chapel, Supplies, Plant Services, Dietary, Support Services, Staff Development, Administration offices and Conference Rooms. The Cottages bear the names of Alaskan rivers and are arranged in alphabetical order along Compassion Circle.
Each cottage is a home that contains 12 private bedrooms/baths situated around a warm, inviting hearth and living area. An open kitchen allows residents and guests to enjoy the aromas and happenings of a kitchen, just like home. Each Cottage includes a Den along with a Spa and beauty/barber shop station. A patio off the dining area provides easy access to the outdoors and gardening opportunities. Large windows in the living area provide views for all to enjoy.
